REQUIRED FOR SPORTS PARTICIPATION
UPDATE: 8/8/2019
Please note that 3 new forms have been added to each student-athlete's account that need your attention. The Urgent Care Intake Form (now electronic) will serve as a replacement for any hard copy to be printed and turned in to the office. These forms will appear red until both parent and student have signed off on them. Those with unsigned forms, including all remaining forms, beginning Monday 8/12/2019, will be held out of practice and competitions until completed.
